Every family’s situation is different.
Some families need respite now. Others are planning for future supported living. Some are looking for community programs, private support, or help understanding what options may be available.
Agnola Care Homes Corp. is being developed to support families through those different stages.
The first step is to complete our Family Interest Form.
We will review information such as:
Age
Diagnosis and support needs
Activities of Daily Living (ADLs)
Communication needs
Behavioural and safety considerations
Mobility and accessibility
Respite needs
Preferred location
Funding
Move timeline
Household compatibility
Family goals
If there appears to be a potential fit, we may contact the family for a more detailed conversation.
Completing the form does not guarantee services, respite, housing, or tenancy.
Our initial supported-living model is being developed as a private-pay and family-funded option.
Families may use a combination of:
Personal income
Family contributions
Trust funds
Eligible individualized funding
Passport funding for eligible supports
ODSP income or shelter funding where applicable
Other approved benefits or resources
Housing costs and support-service costs may need to be billed separately depending on the funding source.
Families should always confirm eligible expenses directly with their funding administrator.
Passport funding may help eligible adults with developmental disabilities purchase certain supports such as:
Community participation
Activities of Daily Living support
Support-worker services
Caregiver respite
Person-directed planning
Passport does not automatically cover ordinary rent, groceries, household expenses, or every type of service.
Families should confirm whether a specific expense is eligible before relying on Passport funding to pay for it.
Families do not need to wait until a crisis happens to begin planning.
You may join our interest list if your family member is:
Looking to move soon
Planning for the next 6–12 months
Preparing for adulthood
Exploring future housing
Interested in respite first
Not sure what level of support will be needed
The information families provide will help us determine where future Agnola homes and services are needed most.
Agnola will consider:
Individual support needs
Staffing requirements
Safety
Household compatibility
Accessibility
Location
Available funding
Family involvement
Property suitability
Applicable municipal and provincial requirements
Whether we can safely meet the person’s needs
Higher support needs do not automatically exclude someone, but the staffing and environment must be appropriate.
